中国肿瘤临床与康复

目的　提高肺癌肉瘤的诊治水平。方法　通过收集我院 2 2年中 1 1例经病理证实的肺癌肉瘤的分析 ,结合文献复习 ,对肺癌肉瘤的组织发生、病理、临床特征、诊断、治疗以及预后进行讨论。结果　肺癌肉瘤是一种罕见的既有癌又有肉瘤两种成分的复合性肿瘤。临床上分为中央型和周围型两种。前者肺肿块位于肺门部 ,生长缓慢 ,转移晚 ;后者肿块位于边缘部 ,生长迅速 ,早期发生转移。结论　肺癌肉瘤常易误诊 ,预后差。首选治疗为手术切除。

Objective To improve the diagnosis and treatment of pulmonary carcinosarcoma.Methods Eleven patients with pulmonary carcinosarcoma collected from our hospital in the past 20 years were confirmed by pathology. By reviewing the relevant literatures,the histogenesis,pathology,clinical features,diagnosis,treatment and prognosis were discussed.Results Pulmonary carcinosarcoma was a rare disease having a mixture of carcinoma and sarcoma.It might be divided into 2 types.One tended to grow slowly in a polypoid pattern along the major bronchi with local invasive feature,the other arising peripherally tended to metastasize early with serious outcome.Conclusion This tumor may be misdiagnosed as brondrogenic carcinoma preoperatively.It tends to metastasize early and the prognosis is poor.Surgical intervention is the first choice of treatment.
